Description
Posterior thigh anatomy is centered on the long head of biceps femoris, arising from the ischial tuberosity and descending along the posterolateral compartment of the thigh. Its muscle belly lies lateral to semitendinosus and semimembranosus, then narrows distally to join the short head and form a common tendon that courses to the lateral side of the knee. At the knee, the tendon inserts primarily on the head of the fibula, with expansions toward the lateral tibial condyle and the deep fascia of the leg, framing the lateral margin of the popliteal fossa. Key landmarks are clear. This posterior perspective matters when teaching hamstring anatomy because biceps femoris is both a hip extensor and a knee flexor, yet it also contributes to external rotation of the flexed knee and stability of the posterolateral corner. Clinically, the proximal tendon near the ischium is a common site of hamstring strain and, less commonly, avulsion in sprinting and kicking sports, while the distal tendon’s relationship to the fibular head puts the common fibular (peroneal) nerve at risk during lateral knee approaches, fibular head fixation, or proximal tibiofibular joint procedures. The long head also serves as a practical surface guide for the course of the sciatic nerve, which runs deep and medial in the posterior thigh before dividing more distally.
