Lateral anatomy of the male thigh and leg frames the long head of biceps femoris as the dominant posterolateral hamstring, arising proximally from the ischial tuberosity and running inferiorly along the posterior femur. Its muscle belly sits posterior to the femoral shaft and posterior to the lateral aspect of the hip joint, then narrows distally toward the fibular head, where the tendon inserts on the lateral side of the proximal fibula. Anteriorly, the quadriceps mass (often represented by rectus femoris or vastus lateralis in this perspective) lies in front of the femur, while the patella, tibia (medial), fibula (lateral), and the tarsals, metatarsals, and phalanges establish the bony landmarks through the knee and ankle. Orientation is clear. Clinically, the long head is a frequent site of hamstring strain during sprinting, with injuries often clustering at the proximal musculotendinous junction near the ischial tuberosity or distally as the tendon approaches the fibular head. That distal insertion matters in posterolateral knee pain and instability: the biceps femoris tendon blends with the fascia near the lateral collateral ligament and sits close to the common fibular (peroneal) nerve as it wraps around the neck of the fibula, a relationship surgeons must respect during lateral knee approaches and proximal fibular fixation. The lateral line of pull also helps explain why avulsions or tightness can contribute to altered tibial external rotation mechanics.
