Description

Cut through the renal parenchyma reveals a peripheral renal cortex surrounding a deeper renal medulla organized into renal pyramids. Renal columns extend inferiorly and superiorly between adjacent pyramids, separating medullary units as they converge toward the renal sinus. Medially, the papillae project into minor calyces, which join to form major calyces and the renal pelvis, continuing inferiorly as the ureter at the hilum region. Cortex outside, collecting system inside. That cortex to medulla transition is the teaching hinge for how filtration couples to concentration: glomeruli and convoluted tubules cluster in the cortex, while loops of Henle and collecting ducts descend through the medulla toward the papilla. This sectional anatomy also maps directly to obstruction and infection patterns, since a calculus lodged at the ureteropelvic junction can dilate the renal pelvis and calyces (hydronephrosis), and ascending pyelonephritis often targets the medulla and papillae, where papillary necrosis may appear in diabetes mellitus, analgesic nephropathy, or sickle cell disease.
