Description
Sectioned through the long axis of a male kidney, the cut surface distinguishes the granular renal cortex from the deeper renal medulla arranged as medullary pyramids with papillae projecting toward the renal sinus. Between pyramids, renal columns extend inferiorly from the cortex, while the renal capsule outlines the convex lateral border and the concave medial border approaches the hilum. Minor calyces cup the papillae and converge into major calyces, which funnel into the renal pelvis at the center of the collecting system. Cortex to papilla is the pathway clinicians teach when explaining hematuria, obstruction, and reflux. This sectional view clarifies where stones lodge, from a minor calyx into the renal pelvis and on toward the ureteropelvic junction, and it helps explain why papillary necrosis (classically in analgesic nephropathy, diabetes mellitus, or sickle cell disease) produces sloughed tips at the medullary papillae with resultant colicky pain and gross hematuria. For nephron physiology, the cortex-medulla gradient becomes intuitive when the pyramids and collecting ducts are shown draining into discrete papillae.
