Description
Running across the palmar (plantar-equivalent) aspect of the male hand, the superficial transverse metacarpal ligament spans the distal metacarpus as a thin fibrous band linking the volar plates at the metacarpophalangeal joints of digits II through V. The heads of the metacarpal bones sit deep to the ligament, with the ligament positioned distal to the metacarpal shafts and proximal to the proximal phalanges. Intermetacarpal spaces open dorsally and superficially, while the ligament lies anterior (volar) to the deep transverse metacarpal ligament and the interosseous muscle bellies. Palmar skin and fascia are removed to expose the ligament’s relationship to the MCP joint line. This palmar view matters because transverse metacarpal restraints define forefooted metacarpal splay and influence the resting cascade of the fingers, a point that becomes obvious in rheumatoid arthritis where volar plate attenuation and ulnar drift alter MCP alignment. It also guides operative planning around the MCP joints, where exposure for synovectomy, collateral ligament repair, or reconstruction must respect the transverse connections that stabilize the distal metacarpal arch.
