Presented from a posterior (dorsal) perspective, the hand is opened to the superficial dorsal venous network spanning the dorsum of the metacarpus and running distally as dorsal digital veins along the fingers. Proximally, these channels coalesce toward the dorsal venous arch near the bases of the metacarpals, continuing as the cephalic vein laterally (radial side) and the basilic vein medially (ulnar side), with accompanying dorsal metacarpal arteries and dorsal carpal branches implied along the extensor compartments. Extensor tendons of extensor digitorum, extensor indicis, and extensor digiti minimi course longitudinally over the metacarpals, while abductor pollicis longus and extensor pollicis longus define the radial border of the anatomical snuffbox. Bony landmarks orient the field. Dorsal hand vascular anatomy is not a diagrammatic nicety, it is where clinicians cannulate, cut, and sometimes injure. The dorsal venous arch is a common site for peripheral venous access, yet its caliber and course can be altered by dehydration, edema, or prior thrombophlebitis, and superficial branches are vulnerable during dorsal approach incisions for metacarpal and phalangeal fixation. This posterior view also clarifies why extensor tendon injuries and lacerations often coexist with venous bleeding on the dorsum, and it sets up the relationship between the snuffbox, the radial artery (deep), and overlying superficial veins used for access or flap planning.
