Description
Positioned in posterior anatomical orientation, the third cervical vertebra (C3) sits inferior to the axis (C2) and superior to C4 within the cervical spine, with its bifid spinous process projecting posteriorly in the midline. Lateral to the spinous process, the paired laminae converge toward the vertebral arch, and the superior and inferior articular processes occupy the posterolateral corners where they contribute to the zygapophysial joints. Transverse processes extend laterally and contain the foramina transversaria, while the vertebral canal lies anterior to the laminae and houses the cervical spinal cord. A posterior perspective on C3 is a clean way to teach the hallmark features of a typical cervical vertebra, namely the short bifid spinous process and the obliquely oriented facet joints that favor rotation and lateral flexion. Palpation and posterior exposure matter clinically: C3 serves as a surface landmark region for cervical tenderness after whiplash, and the posterior elements (lamina, spinous process, facet complex) are the structures evaluated for fractures, perched facets, and alignment on trauma imaging. Small details carry consequences. Facet joint arthropathy at C2 to C3 or C3 to C4 can refer pain to the upper neck and occipital region, and the foramina transversaria relate directly to the vertebral artery’s protected course in the subaxial cervical spine.
