Description
Centered in the mid cervical spine, the C5 vertebra is presented from a posterior aspect, with the bifid spinous process projecting posteriorly and the laminae converging toward the midline. Laterally, the right and left transverse processes extend from the vertebral arch, framing the vertebral foramen and leading toward the transverse foramina that transmit the vertebral vessels in life. Superior and inferior articular processes sit at the junction of pedicle and lamina, their facets oriented to guide flexion and extension while limiting rotation. It is a clean posterior bony read. Posterior C5 anatomy matters when you are teaching cervical biomechanics or planning posterior cervical exposure, because the spinous process and lamina form the working corridor for laminectomy, laminoplasty, and lateral mass screw fixation from C3 to C6. Orientation of the C5 articular pillars helps explain why facet fracture subluxations can produce segmental instability and why foraminal stenosis from uncovertebral and facet arthrosis often localizes symptoms to C5 radiculopathy (deltoid weakness, lateral shoulder pain). Palpable landmarks matter. C5 is a frequent reference level in cervical myelopathy discussions.
