Description
Posterior anatomy places the abdominal viscera in clear relationship to the vertebral column, diaphragm, and posterior abdominal wall in an adult African male. The esophagus descends in the posterior mediastinum to the stomach, which sits left of midline under the left hemidiaphragm, while the liver occupies the right upper quadrant with the gallbladder on its inferior surface. Behind the stomach, the pancreas crosses the upper abdomen retroperitoneally, its head nesting in the C loop of the duodenum to the right of the lumbar spine. Inferiorly, jejunal and ileal loops fill the central abdomen as the colon frames them, with ascending colon on the right, descending colon on the left, and sigmoid colon coursing into the pelvis. Posterior perspective matters because it mirrors how clinicians think about retroperitoneal relationships and referred pain patterns. Pancreatitis, for example, produces epigastric pain radiating to the back because the pancreas lies anterior to the vertebral bodies, and posterior gastric ulcers can penetrate into the pancreas. This orientation also supports safe planning for posterior approaches and interventions, including percutaneous drainage of collections in the lesser sac or pararenal spaces, where the stomach, pancreas, and colon define access corridors.
