Description

Posteriorly, the paired kidneys sit lateral to the vertebral column, each capped superiorly by its suprarenal (adrenal) gland and invested by the renal capsule overlying the renal cortex. The abdominal aorta descends slightly left of midline while the inferior vena cava lies to the right, and the renal arteries and veins course laterally at the renal hila. Near the origin of each renal artery, the renal ganglion appears as a small autonomic nerve cell cluster with nerve fibers tracking along the renal vessels toward the renal plexus. Inferiorly, the ureters exit the renal pelvis and descend on the posterior abdominal wall. A posterior perspective is the working angle for understanding perivascular autonomic innervation of the kidney, because the renal plexus and ganglia follow the renal artery from the aorta and become relevant during hilar dissection. This is where sympathetic fibers that modulate renal blood flow and renin release travel, and it is also where iatrogenic denervation can occur during nephrectomy, renal transplantation, or renal artery endarterectomy and stenting.
