Description
Positioned against the posterior abdominal wall, the left kidney sits lateral to the abdominal aorta with the suprarenal (adrenal) gland capping its superior pole. From a posterior perspective, the renal artery courses laterally from the aorta to the renal hilum, while the renal vein returns medially toward the inferior vena cava, typically lying anterior to the artery at the hilum even when approached from behind. The ureter exits inferiorly from the renal pelvis and descends on the psoas major, and a fine network of periarterial nerves and lymphatics tracks alongside the renal vessels toward paraaortic lymph nodes. A posterior rendering matters because it matches how many retroperitoneal exposures are conceptualized, and it clarifies what lies directly behind the renal pedicle: quadratus lumborum, psoas major, and the costovertebral angle. That relationship underpins common clinical scenarios, from flank pain localization in acute pyelonephritis to the path of a percutaneous nephrostomy tract that aims for a posterior calyx while avoiding segmental arteries that branch early from the renal artery.
