Description
Arising from the lumbosacral plexus (L4 to S3), the sciatic nerve (nervus ischiadicus) exits the pelvis via the greater sciatic foramen and courses inferiorly and laterally in the gluteal region before descending in the posterior thigh. From this posterior perspective, the nerve runs deep to the gluteus maximus and tracks along the posterior aspect of the femur toward the popliteal fossa, where it separates into the tibial and common fibular (peroneal) divisions near the knee. Bony landmarks such as the sacrum, iliac crest, acetabulum, and the proximal femur frame its proximal trajectory, while the distal femur and knee joint orient its bifurcation. Color-coded peripheral nerves and accompanying veins provide relational context in the pelvis and thigh. Clear landmarks. Posterior mapping of the sciatic nerve matters when correlating dermatomal pain with a single trunk lesion versus a root-level radiculopathy. In clinical practice, the close relationship to the posterior hip and proximal femur makes this view relevant to posterior approaches to the hip, deep gluteal syndrome with compression near the piriformis, and iatrogenic injury risk during gluteal intramuscular injection when the superolateral quadrant is not respected. The division into tibial and common fibular components also sets up the classic teaching point: a high sciatic lesion can produce mixed deficits that later mimic isolated tibial neuropathy or common fibular neuropathy depending on fascicular involvement. Ideal use cases include gross anatomy lab teaching on the lumbosacral plexus and posterior thigh, neurology and orthopedics lectures on sciatica patterns and peripheral nerve injury localization, and surgical education materials covering posterior hip exposure and safe injection zones.
