Description
Emerging from the lumbosacral plexus (L4 to S3), the sciatic nerve (nervus ischiadicus) is shown as a thick paired trunk coursing inferiorly from the pelvis into the posterior thigh. Its proximal segment passes posterior to the acetabulum and hip joint, then descends deep to the gluteal region toward the midline of the posterior femur before continuing distally toward the popliteal fossa. Adjacent bony landmarks include the lumbar vertebrae, sacrum, ilium, proximal femur, and the long bones of the leg (tibia and fibula), providing clear orientation for the nerve’s long axis from proximal pelvis to distal knee. Course matters here. The sciatic nerve’s relationship to the pelvis and proximal femur is the anatomic basis for classic patterns of sciatica from L5 or S1 radiculopathy and for less common entrapment sites such as piriformis syndrome, where symptoms can mimic disc disease but the compression is extra-spinal. The depiction of a broad, continuous trunk also supports teaching of iatrogenic risk zones, including posterior hip approaches and deep intramuscular injections in the gluteal region, where injury tends to involve the common fibular component and can present with foot drop. A long nerve with few forgiving margins.
