Centered in the anterior pelvis, the urinary bladder is sectioned in the sagittal plane, with the pubic symphysis lying anterior to the bladder dome and the urethra coursing inferiorly from the bladder neck toward the vestibule. A calcium oxalate dihydrate calculus occupies the bladder lumen, typically settling dependently along the posterior-inferior bladder base near the trigone, where the ureteric orifices sit posterolaterally and the internal urethral orifice marks the outlet. Posterior to the bladder, the uterus and cervix (when present) and the anterior vaginal wall form the immediate soft tissue boundary, with the rectum more posterior still. Hard, faceted mineral. Cystolithiasis in women is less common than in men, so a sagittal female view helps differentiate a true bladder stone from distal ureteral calculi and from calcified pelvic phleboliths that sit outside the urinary tract. Calcium oxalate dihydrate stones are classically radiopaque and often spiculated, a surface texture that correlates with dysuria, urinary frequency, suprapubic pain, and intermittent gross hematuria as the stone abrades urothelium during bladder filling and emptying. This orientation also reinforces how even a small intravesical calculus can intermittently obstruct the bladder outlet at the internal urethral meatus, a pattern seen with neurogenic bladder, foreign bodies, bladder diverticula, or postoperative urinary retention.
