Description
Centered within the vesical lumen lies a calcium oxalate dihydrate calculus, its faceted surface contrasted against the rugae of the bladder mucosa in an anterior sectional (cross-section) view. Superiorly, the dome of the urinary bladder arches toward the peritoneal reflection, while inferiorly the bladder base tapers into the bladder neck and proximal urethra. Posterior to the bladder, the anterior vaginal wall and cervix or uterus would be expected in an adult female pelvis, with the trigone positioned posteroinferiorly between the ureteric orifices and the internal urethral orifice. Orientation is in true anatomical position. Calcium oxalate dihydrate stones are a common crystalline subtype in cystolithiasis and their morphology matters because it correlates with fragmentation behavior during endoscopic treatment. A vesical stone sitting dependently near the bladder base can abrade the urothelium and trigone, driving gross hematuria, suprapubic pain, and irritative lower urinary tract symptoms, and it can intermittently obstruct the bladder outlet to produce stop-start voiding. In female patients, an underlying nidus such as a retained suture, mesh exposure, neurogenic bladder with urinary stasis, or chronic infection often deserves as much attention as the stone itself, since recurrence follows if the cause is missed. Use this illustration in urology and pelvic anatomy teaching to anchor discussions of bladder landmarks (trigone, ureteric orifices, bladder neck) and to differentiate renal calculi from true bladder stones in clinical reasoning.
