Description

Oriented in an anterior sectional plane, the adult female urinary bladder is opened to reveal the lumen and wall layers, with a calcium oxalate dihydrate calculus resting dependently against the posterior-inferior aspect of the cavity. The bladder neck narrows inferiorly into the proximal urethra, while the trigone lies on the posteroinferior base between the expected positions of the ureteric orifices and the internal urethral orifice. Anteriorly, the bladder wall relates to the pubic symphysis across the retropubic space, and posteriorly it approximates the anterior vaginal wall and cervix or lower uterine segment, depending on distension. Mucosa and detrusor thickness can be appreciated around the stone. Calcium oxalate dihydrate stones are a common crystalline type and, when they form as a bladder stone (cystolithiasis), they often indicate urinary stasis rather than a primary renal lithiasis alone. This view is useful for teaching why patients present with terminal hematuria, suprapubic pain, urinary frequency, and intermittent stream that changes with position as the calculus intermittently obstructs the bladder outlet. It also supports procedural planning: transurethral cystolitholapaxy targets the stone within the bladder lumen while protecting the trigone and ureteric orifices, and larger stones may prompt percutaneous or open cystolithotomy in selected cases. Location matters.
