Centered in an anterior sectional (cross-sectional) view of the adult female pelvis, the urinary bladder is opened to reveal its lumen with a rounded cystine calculus resting dependently near the bladder base. Superiorly, the bladder dome tapers toward the urachus remnant, while the posterior-inferior floor outlines the trigone between the ureteric orifices and the internal urethral orifice. Bladder wall layers are read from superficial to deep as serosa/adventitia, detrusor muscle, and mucosa with rugae, and the proximal urethra continues inferiorly from the bladder neck in the midline. A solid intravesical stone. Cystine stones arise in the setting of cystinuria and tend to recur, so seeing a discrete cystolith within the bladder lumen immediately frames the clinical problem as more than routine calcium oxalate disease. This sectional perspective is useful for teaching why bladder outlet obstruction (for example, urethral stricture, neurogenic voiding dysfunction, or postoperative retention) promotes urinary stasis and stone growth at the most dependent portion of the cavity, often with irritative lower urinary tract symptoms and recurrent urinary tract infection. It also supports discussion of endoscopic management, where cystoscopy identifies the calculus relative to the trigone and ureteric orifices before laser cystolitholapaxy, and of imaging pitfalls, since cystine calculi can be less conspicuous on plain radiography than calcium-based stones.
