Description
Framed in a mid-sagittal section of the adult female pelvis, the urinary bladder sits posterior to the pubic symphysis and anterior to the uterus and upper vagina, tapering inferiorly into the bladder neck and short female urethra. A silica calculus is lodged dependently within the vesical lumen, near the posterior-inferior aspect where stones often settle toward the trigone. The detrusor wall outlines the bladder dome superiorly and thickens toward the outlet, with the urethra continuing anteroinferiorly toward the vestibule. Cystolithiasis in women is less common than in men, so a clearly localized bladder stone is a useful teaching setup for secondary causes: chronic urinary stasis, recurrent infection, bladder diverticulum, foreign material, or neurogenic bladder. Location matters clinically, because a mobile calculus can intermittently ball-valve the internal urethral orifice, producing episodic urinary retention, suprapubic pain, terminal dysuria, and hematuria that worsens at the end of micturition. When you are differentiating composition, cystine stones trend toward renal origin and radiolucency, while silica calculi are uncommon and often prompt a history review for exogenous silica exposure, medications, or unusual dietary sources, and the sagittal anatomy helps explain why symptoms can mimic urethral pathology in a short, straight female urethra.
