Description
Superior margin of the cerebral hemisphere curves along the vertex of the cerebrum, forming the upper border where the medial surface turns onto the superolateral convexity. Along this crest, the cortex approaches the interhemispheric (longitudinal) fissure medially, with the falx cerebri occupying the cleft between right and left hemispheres. Neighboring gyri at the crown often include the superior frontal gyrus anteriorly and the upper portions of the precentral and postcentral gyri near the central sulcus, with parasagittal cortex continuing inferiorly onto the medial wall. Midline relationships dominate. Clinically, this margin matters because parasagittal cortex and adjacent venous structures cluster here, so pathology at the vertex can present with leg-predominant motor or sensory deficits from involvement of the paracentral lobule and nearby precentral and postcentral regions. Meningiomas arising from the falx or along the superior sagittal sinus commonly compress the superior medial hemisphere, and bridging veins crossing from cortex to sinus complicate surgical corridors during an interhemispheric approach. The fixed perspective and high-contrast cortical surface detail in the illustration support teaching the turning point between medial and lateral hemispheric surfaces, a frequent source of spatial errors when correlating gross anatomy with parasagittal MRI and CT.
