Presented from an anterior orientation in an adult male, the medial thigh is opened to emphasize the adductor brevis, the short, deep adductor lying inferior to the pubic crest and medial to the proximal femur. Its fibers arise from the body and inferior ramus of the pubis, then pass inferolaterally to insert along the pectineal line and proximal linea aspera, just posterior to the pectineus and deep to the adductor longus. Adductor magnus sits posteriorly and more extensively along the femoral shaft, while gracilis remains the most medial strap, descending toward the pes anserinus. Bony landmarks of the pelvis (pubis, ischium, ilium) and the femur anchor the muscle’s origin and insertion in anatomical position. For groin pain, this plane matters because the adductor brevis is commonly involved in proximal adductor strains yet is often obscured by adductor longus on surface anatomy and on standard teaching diagrams. The interval between adductor longus and adductor brevis is also the corridor for the anterior branch of the obturator nerve, and irritation or entrapment here can mimic hip joint pathology with medial thigh pain and weak adduction. A clear look at the pubic rami and proximal femur helps when explaining why osteitis pubis, pubic ramus fractures, or tendon enthesopathy can refer symptoms into the adductor compartment.
