Description
Seen from the anterior aspect, the L5 vertebra stands out by its broad, slightly wedge shaped vertebral body, positioned inferior to L4 and immediately superior to the base of the sacrum at the lumbosacral junction. Lateral to the body, the pedicles lead into the transverse processes, while the superior and inferior endplates frame the L4–L5 and L5–S1 intervertebral discs. Inferiorly, the sacrum continues as a triangular fused segment with the coccyx aligned in the midline below. The anterior longitudinal ligament would course along the anterior surfaces in vivo, though it is not the focus here. L5 matters because it is the transition vertebra between the mobile lumbar spine and the fixed pelvic ring, and its sagittal orientation sets the stage for lumbosacral shear forces. That mechanical environment explains why L5–S1 disc degeneration, isthmic spondylolisthesis (pars defects at L5), and anterior translation of L5 on S1 show up so often in clinic. A clean anterior perspective also clarifies why anterior lumbar interbody fusion (ALIF) at L5–S1 demands respect for midline anatomy and adjacent great vessels, even when the bony landmarks seem straightforward.
