Description
Centered in an anterior view, the L4 vertebral body appears broad and kidney shaped, with its superior and inferior endplates forming the concave surfaces that face the adjacent intervertebral discs. Flanking the vertebral body, the pedicles project posteriorly toward the vertebral arch, while the transverse processes extend laterally from the junction of pedicle and lamina. Superior and inferior vertebral notches bracket each pedicle, helping define the intervertebral foramen with the neighboring vertebra. Clean bony landmarks. L4 sits at a mechanical and clinical crossroads of the lumbar spine, just superior to the lumbosacral junction where load transfer and shear forces increase. That context makes anterior morphology worth studying when correlating degenerative disc disease at L4 to L5 with osteophyte formation along the vertebral body margins, or when planning an anterior or anterolateral approach (ALIF/OLIF) where the surgeon works directly on the vertebral body and disc space. Pedicle width and body shape also matter for instrumentation planning, since L4 pedicle screw trajectories must respect the lateral cortex and the nearby exiting and traversing nerve roots through the intervertebral foramina.
