Description
Anteriorly, the male lumbar spine is dominated by the broad vertebral bodies of L1 through L5 stacked inferiorly between the lower thoracic vertebrae and the superior surface of the sacrum at the lumbosacral junction. The anterior longitudinal ligament tracks vertically along the anterior cortex of each vertebral body and crosses the intervertebral discs, forming a continuous band from the thoracolumbar region toward S1. Laterally, the vertebral bodies widen into the pedicles and transverse processes, while the iliac crests and upper pelvis frame the distal lumbar segments. Clear midline alignment is the point. An anterior perspective matters because it emphasizes load-bearing anatomy and disc-level relationships without the visual dominance of the spinous processes and laminae seen posteriorly. Clinically, anterior osteophytes and disc space narrowing in lumbar spondylosis are often discussed in relation to the anterior vertebral body margins and the anterior longitudinal ligament, and these features can contribute to stiffness and altered sagittal balance. This angle also supports teaching of approaches that work around the great vessels in anterior lumbar interbody fusion (ALIF), where L4 to L5 and L5 to S1 require careful attention to the ventral lumbar spine and the lumbosacral promontory.
