Description
Centered in an anterior view, the lumbar vertebrae (L1 to L5) are emphasized against the adjacent thoracic spine superiorly and the sacrum inferiorly, with the bodies and intervertebral spaces aligned along the midline. From this perspective, the vertebral bodies dominate the contour, while the pedicles and transverse processes project laterally and the posterior elements, including the spinous processes, remain largely hidden behind the vertebral bodies. The lumbar lordosis is evident as an anterior convexity of the lower back, transitioning toward the sacral promontory at the lumbosacral junction. Cranially, the thoracic segment tapers into the lumbar region; caudally, the sacrum and coccyx form the terminal axis of the vertebral column. Anterior lumbar anatomy matters because the vertebral bodies and disc levels are the working corridor for common approaches such as anterior lumbar interbody fusion (ALIF), where level identification (often L4 to L5 and L5 to S1) and restoration of segmental lordosis drive postoperative sagittal balance. Disc degeneration and endplate changes at the lower lumbar levels contribute to mechanical low back pain, while lumbosacral transitional anatomy can complicate vertebral numbering and lead to wrong-level surgery. Clear appreciation of the lordotic curve also supports teaching pelvic incidence, lumbar lordosis mismatch, and why hypolordosis can accompany muscle spasm or postoperative flatback deformity.
