Description
Seen in lateral profile, the male lumbar spine is isolated from the thoracic cage above and the pelvis below, with L1 through L5 highlighted to contrast against adjacent vertebrae. The vertebral bodies stack anterior to the vertebral arches, and the lumbar lordosis is apparent as the column curves anteriorly before meeting the sacrum at the lumbosacral junction. Thick intervertebral discs occupy the spaces between the vertebral bodies, with the disc height most conspicuous along the anterior margins in this projection. Posteriorly, the spinous processes and the facet (zygapophysial) joints define the posterior column. A true lateral view matters because it is the clearest way to teach load sharing between the vertebral bodies and discs versus the posterior elements, and to relate curvature to sagittal balance. This is the angle clinicians default to when discussing L4 to L5 and L5 to S1 degenerative disc disease, spondylolisthesis, and pars interarticularis stress injury, since anterior translation and disc space narrowing are easiest to conceptualize in profile. Disc height is a story. Loss of it changes foraminal dimensions, facet loading, and symptoms.
