Description
Arising from the posterior surface of the manubrium and adjacent first costal cartilage, the sternothyroid runs superiorly in the anterior neck to insert on the oblique line of the thyroid cartilage, sitting deep to the sternohyoid within the infrahyoid strap muscle layer. From an anterior perspective, its paired bellies flank the midline and lie anterior to the laryngeal framework, with the thyroid cartilage positioned superior to the suprasternal region and inferior to the hyoid complex. Bony landmarks of the head and upper thorax frame the field, including the skull base, cervical vertebrae posteriorly, and the clavicles with the upper ribs laterally. Layered anatomy matters here. Understanding the sternothyroid in situ helps clarify why access to the thyroid gland and anterior larynx so often starts by separating or dividing the midline raphe between strap muscles and then reflecting sternohyoid to reach the deeper plane. During thyroidectomy, tracheostomy, or laryngeal framework surgery, this muscle and its neighbors are retracted to expose the thyroid cartilage, cricoid region, and pretracheal fascia without unnecessary trauma; postoperative anterior neck tightness and dysphagia complaints often track back to strap muscle handling. The anterior view also reinforces the sternothyroid’s role as a laryngeal depressor and its distinction from sternohyoid, which instead targets the hyoid. Different attachments, different mechanics.
