Description
Seen in lateral profile beneath the skin of the anterior neck, the sternothyroid muscle forms a flat infrahyoid strap running superiorly from the posterior surface of the manubrium and adjacent first costal cartilage to the oblique line of the thyroid cartilage. It lies deep to sternohyoid and inferior to the hyoid bone, with its superior fibers approaching the lower laryngeal framework rather than reaching the hyoid itself. Anteriorly, the laryngeal prominence and body of the thyroid cartilage sit just deep to the skin, while posteriorly the cervical vertebrae align behind the pharyngeal airway column. Sternocleidomastoid and the clavicle define the lateral boundary of the field. Depression of the larynx during swallowing and phonation depends on coordinated infrahyoid action, and the sternothyroid is the strap muscle that pulls directly on the thyroid cartilage. That anatomical fact becomes clinically concrete in airway surgery: during tracheostomy or cricothyrotomy extension, the strap muscles are separated in the midline and retracted, and the sternothyroid’s deep plane relative to sternohyoid helps explain why deeper dissection can expose the laryngeal cartilages and pretracheal fascia. Neck pain after laryngeal framework procedures can reflect local strap muscle traction. Small muscle, big landmark. This lateral rendering supports teaching of infrahyoid layers in gross anatomy and head and neck surgery curricula, and it fits well in atlas plates explaining strap muscle planes for thyroidectomy, tracheostomy, and anterior cervical approaches.
