Description
Presented from an anterior (palmar) perspective, the hand is opened to the level just deep to the skin, revealing the fibrous digital sheath and its annular ligaments (A1 through A5 pulleys) as white ring-like bands spanning the volar surfaces of the metacarpophalangeal, proximal interphalangeal, and distal interphalangeal regions. Blue flexor tendons, representing flexor digitorum superficialis and flexor digitorum profundus, run distally within these pulleys from the palm into each digit, held close to the phalanges to prevent bowstringing during flexion. Proximally, the palmar aponeurosis thickens the central palm and sits superficial to the flexor tendon bundles and lumbricals, while the pulley system remains aligned along the midline of each finger, medial and lateral to the digital neurovascular bundles that typically course along the sides of the sheath. Clean anatomy. This anterior dissection-style view matters because annular ligament pathology is common and mechanically specific. Stenosing tenosynovitis (trigger finger) most often localizes at the A1 pulley over the metacarpal head, where tendon swelling and pulley thickening cause painful catching and locking that can be palpated and, in many patients, treated with steroid injection or percutaneous or open A1 release. Pulley rupture, classically involving A2 and A4 in rock climbers, leads to flexor tendon bowstringing and loss of finger flexion strength, a concept best taught when the relationship between pulleys, tendons, and phalanges is explicit. Hand anatomy instructors can pair this plate with surface landmarks to teach the flexor tendon sheath, pulley numbering, and the tendon split of flexor digitorum superficialis in a gross anatomy or upper limb module.
