Description
Anterior to the vertebral canal, the C4 vertebral body sits centrally with its superior endplate showing the characteristic cervical uncinate margins and a shallow saddle-like contour. Lateral to the body, the transverse processes project on either side, framing the transverse foramina, while the pedicles lead posteriorly toward the vertebral arch and the midline spinous process, which is largely obscured from a true anterior view. Adjacent cervical vertebrae and the intervening intervertebral discs are aligned superior and inferior to C4, giving clear reference for segmental level and disc space height. C4 is a frequent landmark in cervical biomechanics because the uncovertebral joints (joints of Luschka) at C3 to C7 guide coupled motion and are common sites for osteophyte formation. Small anterior osteophytes at the C4 vertebral body or uncinate processes can contribute to cervical spondylosis patterns, and when they develop posterolaterally they may narrow the intervertebral foramen, correlating with C4 radiculopathy patterns and segmental neck pain. An anterior perspective also supports teaching of anterior cervical discectomy and fusion (ACDF) planning, where level identification, disc space orientation, and endplate geometry matter for graft or cage seating.
