Description
Posteriorly, the fourth cervical vertebra (C4) sits between C3 superiorly and C5 inferiorly, with its bifid spinous process projecting in the midline and the paired laminae forming the posterior arch. Lateral to the spinous process, the articular pillars rise on each side, bearing the superior articular facets that face superoposteriorly and the inferior facets that face inferoanteriorly to form the C3–C4 and C4–C5 zygapophysial joints. The transverse processes extend laterally, and the posterior tubercles and the margins of the transverse foramina can be appreciated in relation to the pedicles and lateral masses. A posterior perspective makes the facet complex and posterior elements easy to teach and to correlate with axial neck pain, since the cervical zygapophysial joints are common pain generators and frequent targets for medial branch blocks and radiofrequency ablation. C4 also sits at the level where degenerative change, fracture patterns of the posterior elements, or postsurgical alignment can alter the space available for the spinal cord within the vertebral canal.
