Description
Running from the foramen magnum to the sacrum, the external vertebral venous plexus forms a longitudinal meshwork draped on the anterior aspect of the vertebral column, tracking across the cervical, thoracic, and lumbar vertebral bodies and continuing over the sacrum within the pelvis. Paired venous channels course on either side of the midline, interconnected by transverse communicating veins that cross anterior to the vertebrae, while segmental tributaries align with the intervertebral foramina where spinal nerves exit. Superiorly the plexus relates to the skull base, and inferiorly it approaches the pelvic venous network near the sacral promontory. Bony landmarks of the male skeleton frame the vessels, including the skull, vertebral arches and bodies, and the pelvic ring. Anterior visualization of this plexus matters because it explains how valveless vertebral veins provide a low-pressure collateral pathway between the pelvis, thorax, and cranial cavity, bypassing the inferior vena cava when intraabdominal pressure rises. This anatomy underpins Batson plexus spread of metastases, classically from prostate carcinoma to the vertebral column, and it also helps account for vertebral osteomyelitis or epidural venous engorgement in conditions such as IVC obstruction.
