Description

Seen in lateral profile, the external vertebral venous plexus forms a longitudinal, anastomosing network along the posterior cervical region, lying superficial to the laminae and spinous processes from the upper cervical vertebrae toward C7. Venous channels track parallel to the cervical spine and communicate through the intervertebral foramina with internal vertebral venous plexuses, placing them in close relationship to the transverse processes and their foramina. Superiorly, the venous meshwork approaches the suboccipital region beneath the occipital bone, while adjacent osseous landmarks include the mandibular ramus and body anteriorly. Because the external venous plexus is valveless and freely communicates with intracranial and paravertebral venous pathways, it provides a clinically relevant route for hematogenous spread of infection or tumor between the head and neck, spine, and cranial cavity. The lateral view also clarifies why epidural venous engorgement can accompany raised intra-abdominal or intrathoracic pressure and why venous bleeding can be brisk during posterior cervical exposure when paraspinal tissues are elevated. Understanding its relation to the intervertebral foramina helps when teaching cervical radicular symptoms in the setting of space-occupying processes that also distort venous drainage.
