Description
Seen in lateral profile, the calvaria and upper cervical spine are overlaid by the external venous plexus, a superficial vascular network within the scalp and subcutaneous tissues that communicates with the deeper cranial venous system. The plexiform veins course posterior and superior to the auricular region, then descend toward the nuchal area to connect with vertebral venous channels along the cervical vertebrae (C1 to C7). Deep to this vascular layer, the mandible with dentition, occipital squama, atlas and axis, and the remaining cervical vertebrae are presented with their spinous processes and zygapophyseal (facet) joints; cervical nerve roots exit laterally through the intervertebral foramina alongside the vertebral vessels. Part of the scapula anchors the inferior field. Scalp veins matter because they form a low-pressure collateral pathway between extracranial and intracranial circulation, with emissary veins providing a potential route for spread of infection from the scalp into the dural venous sinuses. Bleeding can be brisk. In posterior cervical approaches and occipital nerve blocks, appreciating the relationship of superficial venous channels to the greater occipital nerve as it ascends near the superior nuchal line helps reduce hematoma risk and improves landmark-based teaching. The continuity with the vertebral venous plexus also explains how increased intrathoracic pressure can alter venous drainage patterns in the head and neck.
