Description
Running longitudinally along the posterior vertebral column, the external vertebral venous plexus (plexus venosus vertebralis externus) is rendered as a paired, anastomosing venous network draped over the lower thoracic and lumbar vertebrae and continuing onto the sacrum. Posterior elements are readable in relation to the veins, including spinous processes, laminae, and the dorsal surface of the sacrum, with the lower ribs superiorly and the iliac crests and proximal femora inferiorly framing the field. Segmental vessels and adjacent neural structures appear laterally near the intervertebral foramina, where spinal nerves and their dorsal rami exit and course posterolaterally. Intervertebral discs and vertebral bodies remain central reference points, anchoring the venous channels in a clinically familiar posterior topography. Posterior emphasis on the external plexus matters because these valveless veins communicate freely with the internal vertebral venous plexus and with segmental veins in the thoracolumbar region, creating a low pressure pathway for collateral flow and, at times, an unwanted route for spread. Epidural venous engorgement can contribute to symptoms during pregnancy, portal or caval obstruction, and can increase bleeding during posterior spinal exposure. A surgical reality. The arrangement also underpins classic teaching on hematogenous metastasis to the vertebral column (Batson plexus), where pelvic and abdominal malignancies may seed vertebrae without first passing through the pulmonary circulation. Educators can drop this plate directly into gross anatomy labs, spine modules in orthopedic and neurosurgical training, or radiology teaching on epidural venous anatomy and patterns of vertebral metastasis. It also suits textbook figures on posterior approaches to the thoracolumbar spine and sacrum, where controlling venous plexus bleeding and protecting exiting nerve roots are constant concerns.
