Description
Running along the posterior cervical and upper thoracic region, the external vertebral venous plexus forms a longitudinal network on the outer surfaces of the vertebral column, hugging the laminae and spinous processes and extending laterally toward the transverse processes. Segmental tributaries accompany the spinal nerves through the intervertebral foramina, linking the plexus to the intervertebral veins and radicular veins adjacent to the dorsal root ganglia. Superiorly, venous channels continue toward the suboccipital region where they communicate with the occipital vein and the deep cervical vein, and further cranially with tributaries that can connect to the superficial temporal venous drainage. Because these valveless vertebral veins can conduct flow bidirectionally, this view matters when you need to explain how infection or metastasis reaches the spine without passing through the pulmonary filter. It is the anatomic substrate behind hematogenous spread to the vertebral bodies and epidural space, including prostate carcinoma in men and pyogenic spondylodiscitis, and it also helps clarify why epidural venous engorgement can accompany elevated intra-abdominal or intrathoracic pressure.
