Description
Seen from a lateral perspective in an adult male, the external vertebral venous plexus forms longitudinal venous channels tracking along the anterolateral margins of the vertebral bodies from the upper cervical spine to the sacrum. Segmental connections align with the cervical, thoracic, lumbar, and sacral levels, with vessels running parallel to the vertebral column’s physiologic curves and remaining superficial to the vertebrae rather than within the vertebral canal. In profile, the skull, cervical vertebrae, thoracic cage attachments, lumbar vertebrae, and pelvis provide bony landmarks for interpreting the plexus’ course relative to the occiput superiorly and the sacrum and iliac bones inferiorly. Blue-highlighted vessels stand out against the osseous spine. Lateral visualization matters because the external vertebral venous plexus participates in valveless collateral drainage between the pelvis, abdomen, and thorax, communicating with segmental veins and the azygos system and linking indirectly with internal vertebral venous channels. That anatomy explains clinically relevant spread patterns: vertebral body metastases from prostate carcinoma, pelvic infection, or retroperitoneal disease can track through vertebral venous pathways without passing through the caval system. It also clarifies why paravertebral venous bleeding can be persistent during posterior spinal exposure, lateral approaches to the thoracolumbar junction, or instrumentation when segmental veins and their plexiform connections are disrupted. Expect variability.
