Description
Anterior shoulder anatomy is presented with the coracoid process of the scapula projecting anterosuperiorly over the humeral head and glenoid cavity, with the glenohumeral joint capsule spanning between the glenoid rim and the anatomical neck of the humerus. The subcoracoid bursa sits deep to the coracoid and anterior to the capsule, interposed between the subscapularis tendon and the coracoacromial arch region as it approaches the lesser tubercle. Superiorly the clavicle and acromion define the roof of the shoulder girdle, while the bursal sac occupies a medial to anteroinferior corridor adjacent to the anterior glenoid. Small space. Big symptoms. Clinically, this is the bursa implicated in anterior shoulder pain that mimics rotator cuff or intra-articular disease, because subcoracoid bursitis and subcoracoid impingement often coexist with subscapularis tendinopathy and thickening of the rotator interval. The close relationship to the glenohumeral capsule matters when you teach or perform ultrasound guided injection, since needle trajectory must avoid the long head of the biceps tendon in the bicipital groove and stay anterior to the humeral head while targeting the fluid-filled plane beneath the coracoid. It also frames why coracoid morphology and reduced coracohumeral distance can provoke pain during flexion, adduction, and internal rotation.
