Description

Anteriorly, the glenohumeral joint is presented with the proximal humerus seated against the glenoid cavity of the scapula, while the clavicle runs superomedially toward the sternum and adjacent ribs appear laterally as the thoracic cage margin. Deep to the coracoid region and anterior scapular neck, the subscapularis tendon courses laterally toward the lesser tubercle of the humerus. Interposed between the tendon and the anterior capsule and scapular neck sits the subscapular bursa, a synovial-lined sac positioned to reduce friction as the tendon glides. Small space. Big consequence. Subscapular bursitis becomes clinically relevant when anterior shoulder pain is provoked by internal rotation and forward flexion, and it is often discussed alongside subcoracoid impingement and subscapularis tendinopathy. This anterior perspective clarifies the bursa’s relationship to the joint capsule and to the subscapularis insertion, which helps when teaching why distention can mimic intra-articular pathology and why injections are approached from the anterior shoulder with attention to the coracoid process as a surface landmark. The view also supports discussions of potential communication between the subscapular bursa and the glenohumeral joint, an anatomic variant that can influence how joint effusions track.
