Description
Arising from the ulnar artery at the level of the distal palm, the superficial palmar arch (arcus palmaris superficialis) sweeps laterally in a broad curve deep to the palmar aponeurosis and superficial to the long flexor tendons. From its convexity, common palmar digital arteries run distally in the intermetacarpal spaces and then divide into proper palmar digital arteries along the adjacent sides of the fingers, while a communicating branch toward the radial side completes the anastomosis near the thenar region. Venous channels course superficially in parallel, draining toward the cephalic vein laterally and the basilic vein medially. Orientation matters. Palmar vascular anatomy is where anatomy meets procedure. The superficial palmar arch is the dominant contributor to digital perfusion in many hands, and its relationship to the palmar fascia helps explain why lacerations just distal to the distal palmar crease can bleed briskly and still leave tendons intact. This view supports teaching of collateral flow patterns assessed by the Allen test before radial artery cannulation or harvest, and it frames the arterial targets and hazards during carpal tunnel release, Dupuytren surgery, and repair of digital artery injuries after crush or saw trauma.
