Description
Positioned anteroinferior to the external acoustic meatus, the parotid gland occupies the retromandibular fossa between the ramus of the mandible anteriorly and the mastoid process and sternocleidomastoid posteriorly. From its anterior border, the parotid (Stensen) duct runs horizontally across the masseter, then turns medially to pierce the buccinator before opening into the oral vestibule opposite the maxillary second molar. Superiorly the gland reaches toward the zygomatic arch, while its inferior pole extends toward the angle of the mandible and upper neck. A lateral profile like this also implies the nearby course of the facial nerve as it divides within the gland into temporofacial and cervicofacial divisions, with the retromandibular vein and external carotid artery typically deep to the superficial lobules. This lateral presentation matters when planning parotid surgery because surface landmarks correlate with deeper neurovascular anatomy that cannot be sacrificed. Facial nerve paresis remains the complication surgeons work to avoid. It also supports teaching of parotitis and sialadenosis patterns, and of obstructive sialolithiasis, where ductal kinking over the masseter and narrowing at the buccinator penetration can influence symptom location and imaging interpretation.
