Description
Positioned anteroinferior to the external acoustic meatus, the parotid gland occupies the parotid bed between the ramus of the mandible anteriorly and the mastoid process with the sternocleidomastoid posteriorly, its inferior pole tapering toward the angle of the mandible. A superficial lobe drapes laterally across the masseter, while the deep lobe extends medially through the stylomandibular tunnel toward the parapharyngeal space. The parotid duct (Stensen duct) courses horizontally from the gland’s anterior border over the masseter, then turns medially to pierce the buccinator toward the oral vestibule opposite the maxillary second molar. Neurovascular elements traverse the gland, with the facial nerve forming an intraparotid plexus and the retromandibular vein and external carotid artery lying deeper. This regional arrangement is where anatomy becomes procedure. Parotidectomy hinges on reliable identification of the facial nerve trunk near the stylomastoid foramen and on understanding how its temporofacial and cervicofacial divisions relate to the glandular planes. Nerve injury and Frey syndrome remain practical concerns, and the duct’s superficial course explains why midcheek trauma or iatrogenic injury can produce a salivary fistula, while sialolithiasis more often affects the submandibular system but still appears in the parotid duct in dehydration or altered salivary flow.
