Description
Medial to the mandibular ramus, the parotid gland is presented as an isolated specimen with emphasis on the deep lobe tapering anteromedially toward the parapharyngeal space. The gland’s superficial portion would normally lie posterolateral to the masseter and posterior belly of digastric, while the deep portion passes through the stylomandibular tunnel between the mandible and styloid apparatus. Expect the parotid duct (ductus parotideus, Stensen duct) to arise from the anterior border and course forward across the masseter before turning medially to pierce buccinator opposite the maxillary second molar. Clear topography. A medial perspective matters because it is the deep lobe, not the cheek-facing surface, that explains why parotid disease can present as a parapharyngeal mass with medial displacement of the tonsillar pillar and lateral pharyngeal wall. For surgeons planning a parotidectomy, this angle supports teaching of the “dumbbell” configuration of tumors that extend through the stylomandibular tunnel, and it reinforces where the facial nerve typically divides within the gland even though the nerve itself is not necessarily isolated here.
