Description
Positioned anteroinferior to the auricle, the parotid gland occupies the retromandibular fossa, wrapping over the posterior border of the mandibular ramus and lying superficial to the masseter before tapering toward the angle of the mandible. From its anterior margin, the parotid (Stensen) duct courses horizontally across the masseter, then turns medially to pierce the buccinator en route to the oral vestibule opposite the maxillary second molar. Superficial and deep lobes are implied by the gland’s relationship to the mandible and parapharyngeal space, with the facial nerve branching within the gland substance into temporofacial and cervicofacial divisions. Nearby landmarks include the temporalis superiorly and the sternocleidomastoid posteroinferiorly. A tight neighborhood. Lateral anatomy of the parotid matters because it is where surgeons plan skin incisions and dissection planes for superficial parotidectomy while protecting the facial nerve and its peripheral branches at risk from traction and thermal injury. Acute bacterial parotitis and sialolithiasis typically present with painful preauricular swelling and ductal tenderness, and this view clarifies why obstruction along Stensen duct can refer pain to the cheek and worsen with meals. The same relationships explain how parotid tumors can displace the ear lobule superiorly and why the gland’s deep lobe can produce parapharyngeal bulging despite an apparently modest lateral mass. Useful for head and neck anatomy teaching in dental, medical, and speech pathology curricula, and for surgical atlases covering parotidectomy, facelift (rhytidectomy) planes, and facial nerve preservation. It also supports patient education materials and radiology correlation for ultrasound and contrast-enhanced CT localization of parotid lesions relative to the mandibular ramus and masseter.
