Description
Seen in lateral profile, the male rectum descends along the posterior pelvis, continuing inferiorly from the sigmoid colon and ending at the anal canal. Three transverse rectal folds (Houston valves) project into the rectal lumen as semilunar shelves, typically with a prominent middle fold on the left side and superior and inferior folds more often arising from the right. Posteriorly, the rectum follows the concavity of the sacrum and coccyx, while anteriorly it relates to the prostate and seminal vesicles across the rectovesical fascia. No wasted lines. These rectal folds matter because they segment the ampulla and help support fecal content, and they also create predictable internal landmarks during endoscopy and contrast studies. During flexible sigmoidoscopy or colonoscopy, the valves can mimic pathology or obscure a small lesion, and an overconfident push at a fold is a setup for mucosal trauma or, in a diseased rectum, perforation. Their lateral asymmetry also frames common trouble spots for instrumentation and for interpreting a barium enema, where the folds form transient filling defects that must be distinguished from a true polyp or mass.
