Description
Posterior pelvic anatomy is rendered in lateral profile, following the distal sigmoid colon as it curves inferiorly to become the male rectum and ampulla. Three transverse rectal folds (valves of Houston) project into the lumen as semilunar shelves, stacked superior to inferior and alternating in their attachment from one lateral wall to the other. The peritoneal reflection sits superiorly as the rectum transitions from a mobile sigmoid segment to a more fixed pelvic organ. Mucosa and submucosa form the folds, supported by focal thickening of the circular smooth muscle layer. Transverse rectal folds matter when you are trying to distinguish normal intraluminal anatomy from pathology and when planning instrumentation. During rigid sigmoidoscopy or transanal endoscopic procedures, the folds can arrest an advancing scope, and forceful passage risks mucosal tearing or perforation on the anterior wall near the peritoneal reflection. They also explain common sites where hard stool can lodge in fecal impaction and where a lesion may hide from a straight-line view. Normal. But easy to misread.
