Anterior Presentation of the Uterus and Vagina

Shown from an anterior aspect, the uterus sits superior to the vagina with the cervix projecting inferiorly into the vaginal lumen as the portio vaginalis cervicis. The anterior vaginal fornix forms a shallower recess between the anterior cervix and the anterior vaginal wall, while the posterior vaginal fornix is deeper and curves toward the posterior vaginal wall. Superiorly, the uterine corpus aligns with the cervical canal at the internal os, and inferiorly the external os opens into the vaginal canal. Relationship is clear in anatomical position: bladder and vesicouterine pouch lie anterior to the cervix and lower uterine segment, while the rectouterine pouch (pouch of Douglas) is posterior to the posterior fornix. Depth differences between the fornices matter in both examination and intervention. The posterior fornix provides a close corridor to the rectouterine pouch, a route used for culdocentesis and also a site where endometriosis implants and posterior cul-de-sac adhesions are commonly assessed during pelvic exam and laparoscopy. Cervical dilation, labor mechanics, and placement of vaginal devices all depend on understanding how the cervix protrudes into the birth canal and how the fornices contour around it.
