Sagittal pelvic anatomy is organized around the uterine body and cervix superior to the vagina, with the vaginal canal extending inferiorly toward the introitus and aligned between the bladder anteriorly and the rectum posteriorly. The cervix projects into the vaginal vault, creating anterior and posterior fornices, while the uterine cavity continues through the cervical canal to the external os. The uterine axis typically tilts anteriorly relative to the vaginal axis, an anteverted, anteflexed relationship that becomes clear in profile. Alignment matters. Obstetric and gynecologic procedures depend on understanding how the cervix and vagina relate in the midline pelvis. During speculum examination, IUD insertion, endometrial biopsy, or dilation and curettage, the clinician follows the cervical canal, and an unrecognized retroverted uterus increases the risk of creating a false passage or uterine perforation when instruments are advanced against the uterine flexion. Labor mechanics also track these axes: the birth canal is not a straight tube, and the changing angle between uterus, cervix, and vagina helps explain why fetal head descent and rotation occur in stages rather than as a single trajectory.
