Uterus and Vagina

Description

Anterior pelvic anatomy is rendered with attention to the surface morphology of the uterus and vagina, centering the cervix uteri as it projects inferiorly into the vaginal canal. The portio vaginalis of the cervix is surrounded by the vaginal fornices, with an anterior fornix related to the vesicouterine region and a deeper posterior fornix adjacent to the rectouterine pouch (pouch of Douglas). Superior to the cervix, the uterine body and fundus continue cranially, while the vagina tracks inferiorly toward the vestibule in the midline, posterior to the urinary bladder and anterior to the rectum. Spatial relationships are clear. Seeing the cervical tip in relation to the fornices matters for both examination and procedures: the posterior fornix commonly provides the easiest access for sampling (culdocentesis) or transvaginal needle approaches because it lies closest to the peritoneal cavity at the rectouterine pouch. During childbirth, the cervix must efface and dilate before the fetus can traverse the birth canal, so a clean depiction of the cervical contour and vaginal vault supports teaching of labor mechanics and common causes of dystocia. This is also the geometry clinicians navigate during Pap testing, cervical biopsy, and IUD placement, where cervical orientation and fornix depth affect speculum positioning and instrument trajectory. Use this illustration in gross anatomy and pelvic floor teaching sessions to anchor discussions of uterine version and flexion, vaginal vault anatomy after hysterectomy, and how the fornices guide bimanual examination.
