Description
Bronchial anatomy of the right lower lobe is the focus, centered on the lateral basal segmental bronchus (B9) as it branches distally from the lower lobe bronchus and courses inferolaterally toward the posterolateral parenchyma. Proximal relationships typically include the bronchus intermedius superiorly and the takeoff of the superior segmental bronchus (B6) more posterior and cranial, while adjacent basal segmental bronchi (B7, B8, B10) diverge medially and posteriorly within the basal pyramid. Segmental orientation is reinforced by the expected accompanying pulmonary artery branch running superior to the bronchus and segmental pulmonary veins tracking more intersegmentally, often inferior and anterior to the bronchial lumen. Right sided asymmetry matters. The middle lobe bronchus lies anterior and superior compared with the lower lobe bronchial tree. Clear identification of B9 and its segmental territory supports bronchoscopy navigation, segment directed sampling, and planning limited resections such as right S9 segmentectomy for small peripheral non small cell lung cancers or focal bronchiectasis. Segmental anatomy also guides interpretation of CT distributions in aspiration or infection, where dependent lower lobe basal segments are frequently involved, and helps explain why mucus plugging can produce sharply demarcated atelectasis confined to a single bronchopulmonary segment. A practical landmark. Knowing where B9 sits relative to B10 and the oblique fissure reduces wrong segment entry during endobronchial procedures.
