Detailed Depiction of the Ureteric Orifice (Urinary Bladder)

Description

Seen from the bladder lumen, the right and left ureteric orifices open at the posteroinferior aspect of the urinary bladder within the trigone, each positioned lateral to the internal urethral orifice. The interureteric ridge (interureteric fold) spans transversely between the two openings, and the smooth trigonal mucosa contrasts with the rugae of the surrounding vesical mucosa. Each intramural ureter courses obliquely through the bladder wall from superolateral to inferomedial before terminating at its orifice. Small structure, big consequences. That oblique intravesical course forms the functional antireflux mechanism, so this is the view you want when teaching vesicoureteral reflux and when explaining why a congenitally short submucosal tunnel predisposes to reflux and recurrent pyelonephritis. The relationship of the ureteric orifices to the bladder neck also matters during cystoscopy and transurethral procedures, where identifying and protecting the orifices reduces the risk of iatrogenic ureteral injury, postoperative obstruction, or inadvertent placement of a suture or cautery too close to the intramural segment. Use this illustration in urology and pelvic anatomy teaching to orient learners to the trigone, ureteric orifices, and internal urethral orifice as endoscopic landmarks, or in a nephrology or infectious disease context when linking lower urinary tract anatomy to ascending infection.
